Accident & emergency (A&E) is the hospital department that gives rise to amongst the greatest number of hospital errors.

A&E staff often work under great pressure as well as dealing with critical situations and abusive patients.

It is not the purpose of A&E to offer best treatment but rather provide emergency care and to decide whether to admit or discharge a patient. A&E departments should nonetheless be expert in recognising the symptoms that would indicate various conditions as well as recognising urgency.

We have dealt with numerous cases arising from inadequate investigation of potential orthopaedic injury. We have encountered errors including failure to x-ray, misinterpretation of scans and discharging a patient without a diagnosis. In one case, a patient was taken to A&E by ambulance following an overdose and was left unmonitored and suffered severe burns when she apparently set fire to her clothing.
